Services | OUR EYE EXAMS: We offer Comprehensive Eye Health & Vision Examinations for Adults and Children of All Ages. Dr. Van Horn offers a unique and fast automated and computerized vision exam. Dr. Van Horn is a participant in the InfantSEE program of the American Optometric Association, providing free vision assessments to babies before one year of age.
OUR EYEWEAR OPTIONS: Our optical offers all types of high-tech lens options and a full line of frames from budget to designer styles. You will be pleased with our no-fault "bumper to bumper" warranty on our eyewear. We provide a EXCLUSIVE TWO-YEAR WARRANTY against breakage or defect on nearly all of our eyeglass frames, which is longer than you'll find most places offer. If you have trouble, we will repair them or replace them at no charge to you. (Some frames under budget and discount programs may only be under the manufacturer's warranty.) We also offer a two-year replacement warranty on all scratch-resistant coated plastic, polycarbonate and high-index lenses. You cannot get a better guarantee!
OUR CONTACT LENS PROGRAMS:
As experts in contact lens examinations and fittings, we want our contact lens patients to be comfortable and happy. We offer two special programs for those interested in contact lenses. Our FREE CONTACT LENS CONSULTATION includes a one-week free trial with a pair of disposable or frequent replacement contacts, if the doctor finds these are right for you. Our GUARANTEED SUCCESS PROGRAM offers a no-risk way to try contact lenses with unlimited exchanges or returns for the first 60 days.
EYE HEALTH AND EMERGENCY CARE: We are always available (24 hours) for the treatment of eye disease and injury. We offer LASIK (Laser Correction) vision care and free consultations, treatment for dry eye problems, glaucoma management, and co-management of cataract and other surgery.
APPOINTMENTS AND HOURS:
Patients are seen by appointment except in the case of emergency. We offer convenient evening hours one evening each week. If circumstances require it, other evenings or Saturday or Sunday appointments can be arranged on request. If you need to reschedule an appointment for any reason, please notify our office 24 hours in advance so that we can use that time for another patient.
FEES, PAYMENT PLANS AND INSURANCE:
We ask that all charges be paid at the time services are rendered. We accept personal checks, and major credit cards. You can also arrange easy monthly payments with approved credit. Ask about our 90-days no-interest payment plan. You will find our staff is prepared to review your insurance or vision plan with you. As a convenience to you, we will be glad to file your insurance claims. We now file many claims electronically. In most cases, services rendered by our office will be charged directly to you, and all reimbursements will be made directly to you. We do accept assignment on Medicare and several other insurances. Please call the office if you have any questions about insurance.

Types of Payment Accepted | We accept cash, personal checks, American Express®, MasterCard®, Visa®, Discover®, and Payment plans with approved credit.

Insurance Plans | We participate with VSP  and Medicare. VBA, ECPA, EyeMed, American Benefits, and most other insurances. If you have an insurance or plan for which we are not on the panel, please let us know so we can try to join the panel. If you have a discount program like VisionONE through Cole Vision, we will offer you the discount (we aren't on the panel since we don't work for Cole Vision Corporation.)

Facilities and Equipment | We offer fast and accurate Automated Computerized testing with the Nikon Remote Vision system. Every eye examination includes a visual field screening test with an automated perimeter. We also have a corneal topographer to measure the cornea for irregularities, for contact lens wearers, LASIK surgery, keratoconus patients, and others.
